AGING— Investigations of mechanisms of aging and the genetic basis for differences in aging, attempting
to identify causal relationships between aging and cellular metabolism. This includes specific efforts
to identify genes responsible for the onset of age-related diseases such as familial Alzheimer's disease.
AIOS— Research on vaccines and on other strategies for preventing infections by the Human
Immunodeficiency Virus, and by the organisms which cause the opportunistic infections which are a
hallmark of AIDS (Acquired Immunodeficiency Syndrome).
ARTHRITIS— Immunologic research, using new methods from molecular biology, to uncover how
rheumatic diseases such as rheumatoid arthritis and spondyloarthropathy (spinal arthritis) are triggered
by specific infectious agents.
CANCER CELL BIOLOGY— Research on oncogenes, the proteins for which they code, and the
mechanisms by which they act to transform seemingly normal cells into cancerous ones.
CARDIOVASCULAR DISEASE — Providing the science base and direction for programs in high blood
pressure, cholesterol, and smoking education.
DENTAL DISEASE— Studies of antiplaque and antimicrobial compounds to prevent periodontal diseases
and caries.
DIABETES — Studies aimed at the prevention of the emergence and progression of non-insulin-dependent
(Type II) diabetes in individuals with the inherited tendency.
DIABETIC RETINOPATHY— The Early Treatment Diabetic Retinopathy Study, a clinical trial designed
to determine whether argon laser treatment — used alone or in combination with a low, daily dose of
aspirin— can prevent the progression of diabetic retinopathy in patients still in the early stages of diabetic
eye disease.
HIGH RISK PREGNANCY— Prevention of maternal and infant mortality and morbidity due to risk factors
in pregnancy such as prematurity, intrauterine growth retardation, maternal smoking, alcohol and drug
use, maternal diseases (e.g. hypertension and diabetes), and premature rupture of membranes and
other perinatal events.
NEUROLOGICAL DISORDERS — Basic, clinical, epidemiologic, and behavioral studies in stroke, inborn
errors of metabolism, hearing loss in old age, multiple sclerosis, epilepsy, dementias and Parkinson's
disease to intercept their onset or progress.
NUTRITION AND CANCER— Clinical trials and defined population studies to verify the role of nutrition
in cancer prevention, and to study potential chemopreventive agents and evaluate their toxicity and
risk/benefit relationships.
NUTRITION AND HEART DISEASE— Nutrition research, education, and demonstration programs aimed
at lowering cholesterol, blood lipid, and weight levels; plus the role of sodium, calcium, and nutrients
in controlling high blood pressure.
POLLUTION EFFECTS— Studies to better detect and quantify low-level chronic effects of pollutants,
including carcinogenesis, mutagenesis, teratogenesis, and organ toxicity in animals and man.
SMOKING— Research on school-based prevention -programs, development of self-help cessation
materials, involvement of physicians and dentists in patient education, use of mass media in public
education, programs directed at women and minorities, and smokeless tobacco use.
UROLOGiCAL DISEASE — Studies aimed at prevention of benign prostatic hyperplasia, urolithiasis (kidney
stones), and recurrence of urolithiasis.
VIRUSES AND CANCER— Laboratory and epidemiologic research aimed at clarifying the role that DNA
and RNA viruses play in human cancer, and the development of vaccines and other strategies for
prevention.
OTHER— Osteoporosis; surveillance and control of infectious diseases and the accelerated development
of vaccines against these diseases; accident prevention and injury control; and physical fitness and
exercise.

'Biotechnology is defined as any technique that uses living organisms (or parts of organisms) to make or modify products, to improve plants or animals,
or to develop microorganisms for specific uses. Directly related research is defined as that involving genetic manipulation, cloning of DNA, use of special
techniques to isolate and detect DNA, creation of hybridomas and production of monoclonal antibodies, and computer methods used to analyze DNA
and protein sequences. Broader science base refers to research underlying new biotechnology and includes investigations in genetics, molecular biology,
cell biology, and immunology.
NOTE: The Pharmaceutical Manufacturers Association estimates that additional amounts of at least $239 million for FY 1985 and $267 million for FY
1986 were spent by other Federal agencies for support of biotechnology research, based on data collected in an annual survey.

TECHNICAL NOTES
The NIH Data Book (Basic Data Relating to the National
Institutes of Health) has been published annually since 1961 . The
book is prepared under the direction of the Chief, Program
Analysis Branch, Division of Program Analysis, Office of Program
Planning and Evaluation, Office of the Director. Copies of the
current issue are also available at NIH Information offices.
Detail may not add to totals because of rounding and, where
noted, because of data included in totals only. Supplemental
awards, unless noted, are only counted when the parent is from
prior years' funds, and such supplements, if multiple, are counted
as one. Historical data are often revised.
Sources of the data: Data are derived from NIH records
and studies unless otherwise noted. DPA (see Abbreviations
below) develops the data on national and Federal support for
health R&D from annual surveys and other sources. Other
sources of information are: NIH awards and number of persons
in NIH training programs — Statistics and Analysis Branch, DRG;
NIH obligations, appropriations, and number of full-time-
equivalent training positions (FTTP) — Division of Financial
Management, OA; NIH personnel— Division of Personnel, ORS;
Prevention — NIH Division of Disease Prevention; Nutrition — NIH
Nutrition Coordinating Committee; and U.S. Incidence of AIDS—
Centers for Disease Control. Morbidity and mortality statistics
are compiled by NCHS, with adjustments by NIH.
New NIH components: The Health Research Extension Act
of 1985 (P.L 99-1 58) created a new National Institute of Arthritis
and Musculoskeletal and Skin Diseases (NIAMS), which was for-
mally established on April 8, 1986. P.L. 99-158 also designated
the remaining components of the former National Institute of Ar-
thritis, Diabetes, and Digestive and Kidney Diseases (NIADDK)
as the National Institute of Diabetes and Digestive and Kidney
Diseases (NIDDK). The new Institutes are shown in obligations
tables starting with FY 1986. In trend tables of awards, NIAMS
and NIDDK have been projected back to the beginning of the
trend period based on the NIADDK programs that were allocated
to each new Institute. On April 18, 1986, Secretary Otis R. Bowen,
M.D., announced the creation of another new NIH component,
the National Center for Nursing Research (NCNR). NCNR is in-
cluded with OD for FY 1986 in obligations tables, but is shown
separately for FY 1986 in awards tables.

DEFINITIONS
APPROPRIATIONS— represent an authorization by an act of
Congress that permits Federal agencies to incur obligations and
to make payments out of the Treasury for specified purposes.
An appropriation usually follows enactment of authorizing legisla-
tion. Appropriations do not represent cash actually set aside in
the Treasury for purposes specified in the appropriation act; they
represent limitations of amounts that agencies may obligate dur-
ing the period of time specified in the respective appropriation
acts.
AWARDS — include R&D grants and contracts, research training
grants and fellowships, construction grants, and Medical Library
resource grants. See "Obligations" for explanation of reporting
differences between awards and obligations.
BIOTECHNOLOGY— any technique that uses living organisms
(or parts of organisms) to make or modify products, to improve
plants or animals, or to develop microorganisms for specific uses.
Directly related research is defined as that involving genetic
manipulation, cloning of DNA, use of special techniques to isolate
and detect DNA, creation of hybridomas and production of
monoclonal antibodies, and computer methods used to analyze
DNA and protein sequences. Broader science base refers to
research underlying new biotechnology and includes investiga-
tions in genetics, molecular biology, cell biology, and immunology.
HEALTH R&D— denotes medical and health-related projects,
resources, and general support, but no training or construction.
In health R&D series (pages 1-5), the NIH component contains
certain items not classified as research in obligations tables, such
as cancer control, portions of NLM and NIH direct operations,
and foreign currency funds not directly appropriated to NIH.
OBLIGATIONS — represent amounts of orders placed, grants and
contracts awarded, services received, and similar transactions
during a given period that will require payments during the same
or a future period.
Obligations vs. Awards Reporting— Differences between NIH
"obligations" (Tables 5-12), and "awards" (Tables 13-35), are
caused by differences in the treatment of certain items (cancer
control, scientific evaluation, administering vs. funding BID, etc.)
in the accounting methods used. In obligations tables, cancer
control is separately identified; in awards tables, it is integrated
within various mechanisms. This category accounts for the largest
dollar difference between the two. Obligations tables include
scientific evaluation grants/agreements, while awards tables do
not. In obligations tables, BID data are shown under the BID which
received the appropriation; in awards tables, the data are
presented under the BID with the administering responsibility.
Obligations tables include NIH interagency agreements (amounts
appropriated to NIH but transferred to other Federal agencies
for administration), and exclude funds appropriated to other agen-
cies and transferred to NIH for administration. Conversely, awards
tables include funds appropriated to other agencies but transfer-
red to and administered by NIH, and exclude NIH interagency
agreements.
PREVENTION— includes both prevention research and
prevention-relevant research. The former includes research
designed to yield results directly applicable to interventions for
prevention of disease or progression of detectable but asymp-
tomatic disease. The latter includes studies aimed at elucidating
the chain of causation of acute and chronic diseases, and
therefore includes relevant basic research efforts.
R&D CONTRACTS— include both intramural and extramural
"NOT " contracts and SBIR contracts. For NLM, not all contracts
support strictly research activities.
RESEARCH GRANTS OR R&D GRANTS— include research pro-
ject grants, center grants, and "other research" grants. See Table
26 for classifications. Note that cooperative agreements (U
awards) are included under all three of these categories, and that
RCPAs (K awards) are included under "Other research." NIH's
legislative mandate to support a required number of "research

grants" refers to the total of new, competing continuation, and
competing supplemental research projects .
RESEARCH MANAGEMENT AND SUPPORT— includes
management of extramural programs, ODs of l/RDs, and some
management costs applicable to overall operations.
SCIENTIFIC EVALUATION GRANTS/AGREEMENTS— are
grants and cooperative agreements (R09, P09, U09, K09, and
T09) to chairmen of initial review groups, program project com-
mittees, training committees, and other review groups to provide
funds for the operation of the group. These awards are included
in obligations tables, but are excluded from awards tables.
